1st Application: submit 40 days before the seminar

• ACCREDITATION APPLICATION FORM
• LIST OF OFFICERS
• LIST OF SCHEDULE SEMINAR for the year 2010
• CV OF LECTURERS
• OUTLINE (TOPIC/SEMINAR)
• PROGRAM of the applied seminar (pls. specify the TIME the lecture started and ended.)
• P1000 filing fee

After the seminar (maximum of 15 days after) you should submit the following:

• COMPLETION REPORT
• ATTENDANCE SHEET FORM *
• EVALUATION FORM (sample w/ answer) *
• EVALUATION SUMMARY
• LIST OF LECTURERS with their PRC #

2nd seminar and so on.., submit 40 days before the seminar:

• ACCREDITATION APPLICATION FORM
• CV OF LECTURERS
• OUTLINE (TOPIC/SEMINAR)
• PROGRAM of the applied seminar (pls. specify the TIME the lecture started and ended.)
• P1000 filing fee

After the seminar (maximum of 15 days after) you should submit the following:

* Please note that you have to use the Attendance Sheet Form and Evaluation Form during the seminar.
** Original copies must be submitted to the PRC
*** Please submit all documents in Legal size.
