33rd ASIA PACIFIC DENTAL CONGRESS invites you to the
APDC PRE CONGRESS INVITATIONAL GOLF TOURNAMENT
"Chairman's Cup"

We would like to invite all of you to join us and participate in this prestigious Invitational Golf Tournament that the Organizing Committee of the 33rd Asia Pacific Dental Congress has prepared. It will be held at the World class ALABANG COUNTRY CLUB on April 29,2011. Lots of prizes and surprises are in store for you. Php3,000 for non-members, Php2,000 for Alabang Country Club members and $200 for foreign delegates. Inclusive of the GREEN FEE, TOURNAMENT FEE, BUFFET LUNCH, MULLIGAN, RAFFLES and GIVEAWAYS. There will be a fellowship during the awarding. System 36 will be the tournament format. Please see the attachment for tournament rules. The Philippine Dental Association Comprehensive Healthcare Program Committee is now accepting application for enrollment at a premium amount of Php 11,000.00 for Principal members under MAXICARE. Coverage period is for January 25, 2011 to January 24, 2012 for Php 120,000.00 maximum benefit limit per dreaded disease limit per year. This program is open to all PDA members and their Clinic Staff as Principal Enrollees. Dependents can be enrolled individually in hierarchy for Php 11,649.00.

The Annual Bidding for the next Healthcare Provider for the Coverage Period starting December 15, 2010 to December 14, 2011.
